Over the last year we have expanded our frontline services, welcoming our Community Palliative Care team who provide end of life care to support people during the final days.
Service description:
Our team of Community Palliative Support Workers provide practical nursing care and emotional support for patients in their own home or other place of choice. Their care also extends to the family and carers, providing support and guidance during their difficult time. The team work throughout the evening, when symptoms and support are usually most needed. They have given great comfort and support to individuals and families during the last stages of their illness.
Service delivery:
This service is delivered in the homes of our patients throughout the community.
