Being there when you need us

Our Community Palliative Care team provide hands on end of life care and support for people during the final days of their life.  This service means that our patients are supported to spend their final days and hours at home, often surrounded by family and friends, if they choose to do so.  

Service description
Our team of Community Palliative Support Workers provide practical nursing care and emotional support for patients in their own home or other place of choice.  Their care also extends to the family and carers, providing support and guidance during their difficult time.  The team work throughout the evening/during the night, when symptoms are often more pronoucned and when support is usually most needed.  They have given great comfort and support to individuals and families during the last stages of their illness and ensured that people can spend their final days and hours at home, often surrounded by family and friends.

How to be referred
To be referred to any of our services please ask your GP or District Nurse for a referral. 

Service delivery
This service is delivered in the homes of our patients throughout the community.
