Community Palliative Care team

Being there when you need us

This year we expanded our frontline services, welcoming our Community Palliative Care team who provide end-of-life care to support people during the final days.

Our team of Community Palliative Support Workers provide practical nursing care and emotional support for patients in their own home or other place of choice.  Their care also extends to the family and carers, providing support and guidance during their difficult time.

The team work throughout the evening, when symptoms and support are usually most needed.  They have given great comfort and support to individuals and families during the last stages of their illness.

In December 2010, North Devon Hospice welcomed the Community Palliative Care team, seconded to the hospice for 5 years from the Northern Devon Healthcare Trust.
