Community Nurse team

Most patients are cared for by our team of 9 specialist community nurses who visit patients at home.

What can we offer?

•-          advice on pain and symptom control

•-          emotional and spiritual support

•-          practical support

•-          support for family and loved ones

Each community nurse is linked to a GP surgery within a particular area. Last year our community nursing team made over 5,500 patient visits.

Following referral, each patient is visited by one of our community nurses and a care package is agreed. Visits from our nurses can be reassuring and of great comfort to patients and their families.
