Being there when you need us
Last year we expanded our frontline services, welcoming the hospital based Specialist Palliative Care team. They support individuals who are experiencing cancer and any other life-limiting illness, at any stage in their journey.
Based at North Devon District Hospital, the clinical team work in conjunction with hospice care and support services and allied health professionals to ensure consistency of care.

Symptom Control

For those whose symptoms are difficult to manage in their current care setting, specialist care and support is available for patients, ensuring their symptoms are best managed so they can continue their care at home or in their chosen care setting.

End-of-life care
|
|
For those who stay in the latter stages of their illness, the 24 hour specialist medical care supports each person to the end of their lives.

|
Support for all the family
|
|
The team are there to support all the family along their journey, helping them to address emotional and spiritual concerns related to the patients' illness.
|
|
Assessment for hospice admission
|
|
Some patients visiting the hospital based team will be assessed for hospice admission, to meet their needs along their journey.
