7 day service, 8am – 8pm
We are undertaking an exciting period of development and change within the hospice aimed at making our services more flexible and responsive to need. Our Community Service includes Clinical Nurse Specialists, Registered Nurses and Multi Skilled Healthcare Assistants working together to deliver expert end of life and specialist palliative care to patients and families in their own home. The role of the Community Palliative Care Nurse is new to our service and will provide an additional component to our already busy and respected team.
We are looking for a dynamic, highly motivated, forward thinking first level registered nurse to join our community service as a Community Palliative Care Nurse. Working alongside the Clinical Nurse Specialists and Multi Skilled Healthcare Assistants (MSHCA), you will take responsibility for a caseload of patients who require follow-up and maintenance care at home. You will also have oversight of planned packages of care being delivered by our team of MSHCA, supporting them to safely deliver personal care at home throughout our 7 day service. Your clinical knowledge and assessment skills will be used to support patients who call for urgent assistance. You will work across the full week and week-ends be responsible for supporting the MSHCAs.
Experience of working in the community and / or end of life care in any setting would be advantageous but we will support you to develop your skills and knowledge in this area. You will need to be able to work within a busy and close knit team whilst also demonstrating a degree of autonomy in your practice. You will be committed to person centred care and evidence based practice.
We support continued professional development within the team and you will be encouraged to seek out opportunities to learn and develop. We will provide a tailored induction and training programme to support your needs and integration into our service. You must be able to travel across our geography and be able to deliver bedside care.
