Hospice nurses in the community

When the hospice began in 1984, our care and support was provided in people's own homes. Now, over 30 years on, it is still where the majority of our care takes place. We have a team of Clinical Nurse Specialists who visit patients and their families in their own homes, wherever they may live in North Devon.

Our team of hospice nurses have visited people in their own homes for over 30 years. Although a lot has changed over those 30 years, one thing remains very much the same, it is still where the majority of our care takes place.
Our hospice nurses visit patients in their home, offering extra care and information to support patients and their families through their illness. Some of our patients choose to access a range of other care and support services we provide, in addition to this support at home.


Service description
With the vast majority of our patients choosing to be supported at home, our team of nurses work closely with patients and carers, offering them support. Following a referral from a healthcare professional, with the agreement of your GP, each patient will be contacted by one of our hospice nurses and we will explore with you what your needs are, and how we can support you. Support from our hospice nurses can be reassuring and offer a real comfort to patients and their families. It is also an opportunity to access support and advice on:
  • Pain and symptom control
  • Emotional and spiritual support
  • Practical support
  • Support for family and loved ones
  • Information about other hospice services.
Every hospice nurse is linked to a GP surgery within a particular area. The team work alongside 22 local GP practices and other health and social care colleagues in primary, secondary and tertiary settings throughout North Devon. There is always a Clinical Nurse Specialist available to speak to Monday - Friday 8.30am-5pm.
Unlike other healthcare professionals, our hospice nurses don’t wear a uniform and they won’t arrive at a patient’s home in a hospice car. We do this to respect the privacy and wishes or our patients at a difficult time and to ensure that neighbours will just think of us as a visitor.


How to be referred
To be referred to any of our services please ask your GP for a referral.

Service location
The hospice’s team of nurses provide outreach care and support throughout North Devon. Even if you live in a remote and rural location we will be able to come to you. The vast majority of their care is delivered to people in their own homes. Our hospice nurses visit patients in any setting including Hospitals and care homes.
